Influence of Primary Healthcare Accreditation on Quality of Care in Nigerian Primary Healthcare Centres

 

Abstract

Quality of care remains an important concern in Nigeria's primary healthcare system, particularly as primary healthcare centres serve as the first point of contact between individuals and the formal healthcare system. Primary healthcare centres provide essential services such as maternal and child healthcare, immunization, disease prevention, health promotion, treatment of common illnesses, and management of basic health conditions. However, challenges including inadequate infrastructure, shortages of healthcare personnel, limited availability of essential medicines and equipment, poor infection prevention practices, weak record-keeping systems, and variations in adherence to clinical standards may affect the quality of care provided at these facilities. Primary healthcare accreditation provides a structured mechanism for assessing healthcare facilities against established standards relating to infrastructure, service delivery, patient safety, healthcare personnel, infection prevention, documentation, essential medicines, equipment, and clinical practices. Accreditation may encourage healthcare facilities to comply with established standards, identify deficiencies, improve service delivery processes, and strengthen continuous quality improvement. However, limited implementation, inadequate resources, weak monitoring systems, and inconsistent compliance with accreditation requirements may reduce its effectiveness in improving healthcare quality. Against this background, this study investigates the influence of primary healthcare accreditation on quality of care in Nigerian primary healthcare centres. The study will be anchored on Donabedian's Quality of Care Model, Health Systems Framework, and Continuous Quality Improvement Theory. Donabedian's Quality of Care Model explains healthcare quality through the dimensions of structure, process, and outcome, providing a suitable framework for assessing how accreditation standards influence the organization and delivery of healthcare services. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and governance as essential components of an effective healthcare system. Continuous Quality Improvement Theory emphasizes systematic and ongoing efforts to identify deficiencies, implement corrective measures, monitor performance, and improve healthcare processes and outcomes. Collectively, these theoretical perspectives provide a suitable framework for explaining how primary healthcare accreditation may influence quality of care in Nigerian primary healthcare centres. The study will adopt a quantitative cross-sectional analytical research design. The study population will comprise selected primary healthcare centres and healthcare workers providing services in these facilities across Nigeria. A multistage sampling technique will be used to select states, local government areas, primary healthcare centres, healthcare workers, and eligible patients or service users where applicable. Primary healthcare accreditation will be measured using indicators such as accreditation status, compliance with accreditation standards, frequency of accreditation assessment, infrastructure standards, availability of essential medicines and equipment, infection prevention standards, healthcare personnel requirements, documentation standards, patient safety practices, and implementation of recommendations from accreditation assessments. Quality of care will be assessed using indicators such as adherence to clinical guidelines, availability and utilization of essential healthcare services, patient safety practices, infection prevention and control, waiting time, continuity of care, patient satisfaction, healthcare worker performance, accuracy of medical records, and availability of essential medicines and equipment. Data will be collected using structured questionnaires, accreditation assessment records, facility observation checklists, healthcare worker records, patient records, service utilization registers, and relevant programme documents. Descriptive statistics will be used to summarize the characteristics of primary healthcare centres, accreditation status, and quality-of-care indicators. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the influence of primary healthcare accreditation on quality of care. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that primary healthcare accreditation has a significant positive influence on quality of care in Nigerian primary healthcare centres. Accredited or highly compliant primary healthcare centres are expected to demonstrate better adherence to clinical standards, improved infection prevention practices, stronger patient safety systems, better documentation, improved availability of essential medicines and equipment, and more organized service delivery processes than facilities with limited accreditation compliance. Accreditation may also encourage healthcare workers and facility managers to identify deficiencies and implement corrective measures that improve the quality and safety of healthcare services. Regular assessment and monitoring may further promote continuous quality improvement and accountability within primary healthcare facilities. Conversely, inadequate infrastructure, insufficient funding, shortages of healthcare personnel, limited essential medicines and equipment, poor implementation of accreditation recommendations, and weak monitoring may reduce the effectiveness of accreditation in improving quality of care. The study therefore expects effective implementation and sustained compliance with primary healthcare accreditation standards to contribute significantly to improved quality of care in Nigerian primary healthcare centres. The study is expected to contribute to the literature on healthcare accreditation, quality of care, primary healthcare, health systems strengthening, patient safety, healthcare quality improvement, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, National Health Insurance Authority, state ministries of health, primary healthcare development agencies, healthcare facility managers, healthcare workers, accreditation bodies, development partners, and policymakers regarding strategies for improving the quality of primary healthcare services. The study will also provide evidence-based recommendations for strengthening accreditation systems, improving compliance with primary healthcare standards, increasing investment in healthcare infrastructure, ensuring adequate availability of essential medicines and equipment, strengthening healthcare workforce capacity, improving infection prevention and patient safety practices, strengthening monitoring and follow-up of accreditation recommendations, and promoting continuous quality improvement across primary healthcare centres in Nigeria.
