Influence of Public Health Emergency Education on Outbreak Prevention Practices among Community Health Workers in Nigeria

Abstract

Public health emergencies and infectious disease outbreaks remain important threats to population health in Nigeria, particularly in communities where limited healthcare resources, inadequate surveillance capacity, population movement, poor sanitation, and delayed recognition of disease threats may facilitate the spread of infectious diseases. Community health workers play an important role in public health emergency preparedness and response because they often serve as a link between healthcare facilities and communities and may participate in disease surveillance, health education, community mobilization, early case identification, referral, contact tracing, and implementation of preventive measures. Public health emergency education provides community health workers with knowledge and practical skills relating to outbreak recognition, disease surveillance, early warning systems, infection prevention and control, risk communication, case identification and referral, contact tracing, community engagement, reporting procedures, and appropriate response to public health emergencies. Outbreak prevention practices refer to actions undertaken by community health workers to reduce the occurrence and transmission of infectious disease outbreaks, including early identification and reporting of suspected cases, community health education, promotion of preventive behaviours, appropriate infection prevention practices, contact tracing, referral of suspected cases, risk communication, and compliance with established outbreak prevention guidelines. In Nigeria, inadequate emergency preparedness training, limited access to current outbreak information, shortages of essential resources, weak surveillance systems, heavy workloads, inadequate supervision, and inconsistent implementation of outbreak prevention procedures may affect the capacity of community health workers to prevent and respond to disease outbreaks. Effective public health emergency education may improve their knowledge, skills, confidence, and adherence to recommended outbreak prevention practices. Against this background, this study investigates the influence of public health emergency education on outbreak prevention practices among community health workers in Nigeria. The study is anchored on Social Cognitive Theory, the Health Belief Model, and Protection Motivation Theory. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, behavioural capabilities, social influences, and environmental conditions in shaping preventive health behaviour. The Health Belief Model explains preventive health behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding community health workers' adoption of recommended outbreak prevention practices. Protection Motivation Theory explains how perceived health threats, response efficacy, self-efficacy, and perceived response costs influence individuals' adoption of protective behaviours during health emergencies. Collectively, these theoretical perspectives provide a suitable framework for explaining how public health emergency education may influence outbreak prevention practices among community health workers in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from community health workers in selected primary healthcare facilities and communities in Nigeria using a structured self-administered questionnaire, public health emergency knowledge assessment, and an observational outbreak prevention practices checklist. A multistage sampling technique will be employed to select states, local government areas, primary healthcare facilities, communities, and eligible community health workers. Where feasible, selected community health workers or groups will be assigned to intervention and comparison groups. Community health workers in the intervention group will receive structured public health emergency education covering outbreak recognition, disease surveillance and reporting, early warning systems, infection prevention and control, risk communication, early identification and referral of suspected cases, contact tracing, community mobilization, health promotion, emergency communication, and appropriate response procedures. Outbreak prevention practices will be assessed using indicators such as timely identification and reporting of suspected cases, community health education, promotion of preventive behaviours, appropriate infection prevention practices, contact tracing activities, referral practices, risk communication, and adherence to outbreak prevention guidelines. Descriptive statistics will be used to summarize community health workers' sociodemographic and professional characteristics, public health emergency knowledge, exposure to education, and outbreak prevention practices. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and logistic or multiple regression analysis, will be used to determine the influence of public health emergency education on outbreak prevention practices. Where appropriate, pre-intervention and post-intervention practice scores will be compared. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that public health emergency education has a significant positive influence on outbreak prevention practices among community health workers in Nigeria. Community health workers exposed to structured emergency education are expected to demonstrate improved knowledge of outbreak risks and greater adherence to recommended prevention and response procedures. Education may improve early recognition and reporting of suspected cases, community disease surveillance, infection prevention practices, contact tracing, health education, risk communication, referral procedures, and community mobilization during disease outbreaks. It may also improve community health workers' confidence in responding to emerging infectious disease threats and communicating appropriate preventive information to community members. However, education alone may not guarantee sustained outbreak prevention practices because community health workers may continue to face barriers such as inadequate personal protective equipment, shortages of essential supplies, weak surveillance infrastructure, limited transportation, heavy workloads, staff shortages, inadequate supervision, and delayed access to laboratory and referral services. Consequently, public health emergency education is expected to be most effective when supported by functional disease surveillance systems, adequate emergency supplies, regular supervision, accessible referral and laboratory services, effective communication systems, and strong public health emergency preparedness structures. The study is expected to contribute to the literature on public health emergency preparedness, outbreak prevention, community health, disease surveillance, infection prevention and control, health education, community health worker capacity, and public health in Nigeria by providing empirical evidence on the influence of public health emergency education on outbreak prevention practices among community health workers. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Nigeria Centre for Disease Control and Prevention, National Primary Health Care Development Agency, state ministries of health, primary healthcare development agencies, local government health authorities, community health worker programmes, public health practitioners, development partners, and policymakers regarding strategies for strengthening community-level outbreak prevention and emergency preparedness. The study will also provide evidence-based recommendations for strengthening public health emergency education, improving community health worker training and supervision, enhancing disease surveillance and reporting systems, ensuring availability of essential emergency supplies, strengthening risk communication, and improving community-level preparedness and response to infectious disease outbreaks in Nigeria.
