Influence of Vaccine Safety Education on Childhood Vaccination Acceptance among Parents in Nigeria

Abstract

Childhood vaccination is a fundamental public health intervention for preventing vaccine-preventable diseases, reducing childhood morbidity and mortality, and protecting communities from outbreaks. Parents play a central role in deciding whether children receive recommended vaccines and therefore their understanding and acceptance of vaccination are important for achieving high immunization coverage. Vaccine safety education provides parents with accurate information about vaccine benefits, common and expected side effects, vaccine safety monitoring, appropriate responses to minor adverse reactions, contraindications and precautions, vaccine misinformation, and the importance of following recommended childhood vaccination schedules. Childhood vaccination acceptance refers to parents' willingness, intention, and readiness to allow their children to receive recommended vaccines in accordance with the national immunization schedule. In Nigeria, vaccine hesitancy and delayed vaccination may be influenced by misinformation, concerns about vaccine safety and adverse reactions, mistrust, cultural beliefs, religious and social influences, previous experiences with healthcare services, inadequate access to reliable health information, and concerns about vaccine effectiveness. Effective vaccine safety education may improve parents' knowledge, confidence, trust, and willingness to accept recommended childhood vaccination. Against this background, this study investigates the influence of vaccine safety education on childhood vaccination acceptance among parents in Nigeria. The study is anchored on the Health Belief Model, Social Cognitive Theory, and the Knowledge-Attitude-Practice framework. The Health Belief Model provides a framework for understanding parents' vaccination decisions through perceived susceptibility to vaccine-preventable diseases, perceived severity, perceived benefits of vaccination, perceived barriers, and cues to action. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, social influences, and environmental conditions in shaping vaccination-related behaviour. The Knowledge-Attitude-Practice framework provides a basis for examining how vaccine safety education may improve parents' knowledge and attitudes and influence acceptance of childhood vaccination. Collectively, these theoretical perspectives provide a suitable framework for explaining how vaccine safety education may influence childhood vaccination acceptance among parents in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from parents or primary caregivers of children eligible for routine childhood vaccination in selected communities, public and private healthcare facilities, immunization clinics, and child-health programmes in Nigeria using a structured interviewer-administered questionnaire and a standardized childhood vaccination acceptance assessment. A multistage sampling technique will be employed to select states, local government areas, communities, healthcare facilities, and eligible parents. Where feasible, selected communities, healthcare facilities, or participant groups will be assigned to intervention and comparison groups. Parents in the intervention group will receive structured vaccine safety education covering vaccine-preventable diseases, vaccine benefits, vaccine safety, common and expected side effects, vaccine safety monitoring, appropriate responses to minor adverse reactions, contraindications and precautions, vaccine misinformation, and the importance of completing recommended childhood vaccination schedules. Childhood vaccination acceptance will be assessed using indicators such as willingness to vaccinate eligible children, intention to receive recommended vaccines, readiness to follow the immunization schedule, confidence in vaccine safety, and willingness to recommend routine childhood vaccination to other parents. Where feasible, acceptance findings will be compared with actual vaccination records to distinguish stated acceptance from vaccination behaviour. Descriptive statistics will be used to summarize participants' sociodemographic characteristics, previous vaccination experiences, exposure to vaccine information, vaccine knowledge, safety concerns, and vaccination acceptance scores. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and logistic regression analysis, will be used to determine the influence of vaccine safety education on childhood vaccination acceptance. Where appropriate, acceptance before and after the educational intervention will be compared. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that vaccine safety education has a significant positive influence on childhood vaccination acceptance among parents in Nigeria. Parents exposed to structured vaccine safety education are expected to demonstrate improved understanding of vaccine benefits, common side effects, safety monitoring, contraindications, and the importance of completing recommended vaccination schedules. Education may reduce misconceptions and fear surrounding vaccination and increase parents' confidence in the safety and effectiveness of recommended childhood vaccines. Clear explanations from trusted healthcare professionals may also improve parents' willingness to discuss vaccine concerns and make informed vaccination decisions. However, education alone may not eliminate vaccine hesitancy because parents may continue to encounter misinformation through social media and other information channels, mistrust of health institutions, cultural influences, religious beliefs, previous negative experiences, and concerns about vaccine availability or healthcare services. Consequently, vaccine safety education is expected to be most effective when supported by transparent communication, trusted healthcare professionals, reliable vaccine availability, responsive healthcare services, community engagement, and effective systems for addressing vaccine-related concerns and misinformation. The study is expected to contribute to the literature on vaccine safety, childhood vaccination, vaccine acceptance, vaccine hesitancy, health education, parental decision-making, community health, maternal and child health, and public health in Nigeria by providing empirical evidence on the influence of vaccine safety education on childhood vaccination acceptance among parents. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state primary healthcare agencies, hospitals, primary healthcare centres, immunization programmes, nurses, community health workers, public-health educators, development partners, and policymakers regarding strategies for improving parental confidence in childhood vaccination. The study will also provide evidence-based recommendations for strengthening vaccine safety communication, integrating vaccine education into routine maternal and child health services, training healthcare workers in effective risk communication, addressing vaccine misinformation, improving community engagement, establishing responsive mechanisms for addressing parental concerns, and promoting informed acceptance and timely uptake of recommended childhood vaccines in Nigeria.
