Influence of Waiting Time on Patient Satisfaction in Public Hospitals in Nigeria

 

Abstract

Waiting time is an important indicator of healthcare service efficiency and may significantly influence patients' experiences and perceptions of healthcare quality. In public hospitals, patients may spend considerable periods waiting for registration, consultation, laboratory investigations, diagnostic procedures, pharmacy services, payment, and other healthcare processes. Prolonged waiting time can create frustration, inconvenience, anxiety, and dissatisfaction, particularly when patients are not adequately informed about delays or when healthcare services are perceived as inefficient. Waiting time refers to the period patients spend between arrival at a healthcare facility and the receipt of specific healthcare services, including registration, consultation, laboratory investigations, pharmacy services, and discharge. Patient satisfaction refers to the extent to which patients' expectations and perceptions regarding healthcare services are met, including satisfaction with provider interaction, service efficiency, waiting time, communication, facility environment, cost, and overall quality of care. In Nigeria, long waiting times in public hospitals may be associated with high patient volume, inadequate staffing, inefficient registration systems, limited diagnostic capacity, shortages of equipment, administrative delays, and weak patient-flow management. Understanding the relationship between waiting time and patient satisfaction is therefore important for improving healthcare quality and responsiveness in public hospitals. Against this background, this study investigates the influence of waiting time on patient satisfaction in public hospitals in Nigeria. The study is anchored on Donabedian's Structure-Process-Outcome Model, Expectancy-Disconfirmation Theory, and Andersen's Behavioral Model of Health Services Use. Donabedian's Model provides a framework for understanding how healthcare structures and service processes, including patient-flow efficiency and waiting time, influence patient outcomes and experiences. Expectancy-Disconfirmation Theory explains patient satisfaction as the result of comparing expectations about healthcare services with actual experiences, including the amount of time spent waiting. Andersen's Behavioral Model provides a framework for understanding healthcare utilization and experiences in relation to patient characteristics, enabling resources, and healthcare needs. Collectively, these theoretical perspectives provide a suitable framework for explaining how waiting time may influence patient satisfaction in Nigerian public hospitals. The study will adopt a quantitative cross-sectional research design. Primary data will be collected from patients attending selected public hospitals in Nigeria using a structured interviewer-administered or self-administered questionnaire and a patient waiting-time assessment form. A multistage sampling technique will be employed to select states, public hospitals, departments or service units, and eligible patients. Waiting time will be objectively assessed using indicators such as total time spent in the hospital, registration waiting time, time before consultation, laboratory waiting time, pharmacy waiting time, payment waiting time, and time between different stages of service delivery. Where feasible, actual arrival and service times will be recorded through direct observation, hospital registers, electronic queue systems, or time-tracking forms rather than relying exclusively on patients' recall. Patient satisfaction will be assessed using indicators such as satisfaction with waiting time, provider communication, staff responsiveness, cleanliness, service efficiency, availability of services, cost of care, privacy, and overall satisfaction with the hospital experience. Descriptive statistics will be used to summarize patients' sociodemographic characteristics, healthcare service use, waiting-time patterns, and satisfaction levels. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the influence of waiting time on patient satisfaction. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, multicollinearity, and robustness of the findings. The study is expected to find that prolonged waiting time has a significant negative influence on patient satisfaction in public hospitals in Nigeria. Patients who experience longer waiting periods are expected to report lower levels of satisfaction with healthcare services, particularly when delays occur without adequate explanation or communication. Shorter and more predictable waiting times are expected to improve patients' perceptions of service efficiency, responsiveness, and overall quality of care. Long waiting periods for registration, consultation, laboratory services, pharmacy services, and payment are expected to contribute substantially to dissatisfaction. However, waiting time alone may not fully determine patient satisfaction because patients' experiences may also be influenced by provider behaviour, quality of clinical care, availability of medicines, cleanliness, cost of services, staff attitudes, communication, and perceived treatment outcomes. Consequently, reducing waiting time should form part of a broader strategy for improving patient-centred healthcare delivery. The study is expected to contribute to the literature on waiting time, patient satisfaction, healthcare quality, public hospitals, healthcare management, patient experience, and health-services delivery in Nigeria by providing empirical evidence on the influence of waiting time on patients' perceptions of healthcare services. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, public hospital administrators, healthcare managers, healthcare providers, quality-improvement teams, health-service researchers, public health practitioners, and policymakers regarding strategies for improving service efficiency and patient-centred care. The study will also provide evidence-based recommendations for strengthening appointment and queue-management systems, improving patient-flow processes, increasing staffing where necessary, reducing administrative delays, strengthening laboratory and pharmacy service coordination, introducing appropriate digital registration systems, improving communication about delays, and establishing routine waiting-time monitoring mechanisms in Nigerian public hospitals.
